Each person who volunteers or is on staff at Twin Peaks during the Summer must have a Staff Health History Form filled out. This includes the underaged children of volunteers and staff. In the online staff application, you will need to upload this file. Please download this form by clicking on the “Download” link below. You have two options to till it out: You can open the form in Adobe Reader, fill it out, print it, sign it, and then scan it or take a picture of it to upload into your application in the family portal. Or you can print it out, fill it out by hand, sign it, and then scan it or take a picture of it to upload into your application on the family portal.
